The surgery is contracted to the NHS to provide a range of health services to patients. A number of other services are frequently requested by patients which are not an NHS service, and these therefore attract a private medical fee, payable in advance, and a notice period is usually required. The acceptance of any non-NHS work, such as those listed below and on the Table of Fees, is at the discretion of the practice.
Fee levels are reviewed on a regular basis and patients are advised to check the current fee on the Table of Fees, which can be accessed from this page, before requesting a service.
Historically, private fee levels have been recommended by the British Medical Association, but in recent years the fees have been set by individual practices. We believe that our fees are among the lowest in the area. Any enquiries relating to private services and fees may be directed to our private services administrator, Mrs L Freeman.

Medical reports for employment purposes should be requested via your employer. Self-employed patients (only) may make direct contact with the practice. Fees vary according to whether a full examination is required, or whether a report may be prepared from medical records only.
Fees are payable in advance, and enquiries should be made via reception at least 4 weeks ahead. A full list of fees for medical reports of all kinds is listed on the fees table which may be accessed from this page.

Copies of Medical Records |

You may view or obtain copies of your medical records under the provisions of the Data Protection Act 1998, for which a fee is payable. You will need to apply in writing (a form is available at reception) and access will normally be granted within 21 working days of the request. There is no facility for immediate access.
The practice has now teamed-up with an organisation called HealtheCard who can provide you with a credit card sized smartcard, which securely holds your full medical record, viewable on any PC, for a small annual charge. You are able to upload your medical records from the practice database onto your card regularly throughout the year. This arrangement is ideal for patients who travel or stay away from home regularly, visit hospital often, see the doctor several times a year, or who may have significant medical conditions or needs.

Vaccinations provided as an NHS service do not generally include those related to foreign travel. A list of those fees which the practice offers as a private medical service are given in the private fees table accessible from the bottom of this page. Please also see our pages on Travel Advice and Vaccinations.
You are advised to see our Practice Nurse about the best vaccination schedule as soon as your travel arrangements are known.

The practice will respond to requests from the local authority for health information in relation to patients applying for parking concessions. The fees incurred are normally paid direct by the local authority.

Sick Notes (MED3 or MED5 forms) record the advice given to patients regarding their ability to undertake their normal occupational activity, and enable them to obtain financial support where appropriate. These certificates are available as an NHS service but a strict criteria is applied before they can legally be issued. More details of the rules for the issue of these notes can be obtained on our Sick Note Rules page.
Where a note from the doctor is required for a purpose not connected to the patient’s normal occupation (e.g. school absences etc) this may be undertaken as a private medical service and a fee is payable.

Our doctors are often asked to countersign passport applications. With the recent tightening of passport rules there is now a significant personal liability falling on doctors who certify applications on behalf of patients.
Passport certification is not an NHS service, and in common with other categories of private work, each individual doctor within the practice will decide on a patient-by-patient basis whether they are able to legally and honestly certify a passport application. Requests from patients may often be refused.
