There are a number of things you can do on-line, without having to contact the surgery, and these are listed in the menu options on this page. All patients can respond to our Health Questionnaire, our Service Questionnaire and can submit our Suggestion Form. Other forms will become available from time to time.

A number of appointments have been made available for booking on-line, 24 hours a day. These may not be the only appointments available for the day however, and if you are unable to book a convenient appointment on-line, or locate an appointment for the doctor or nurse of your choice you should contact reception in the usual way.

You may order your regular repeat prescriptions on-line, 24 hours a day. The service is limited to regular repeat prescriptions, only. Where this service is used and the request results in a query you may receive a telephone call. The prescription will be available after 2.30pm on the next working day for on-line requests made prior to midnight.

Use our on-line facilities to advise us of changes to your personal details such as telephone numbers, addresses etc. Note that updates to these details will be subject to a post-submission validation process. Any new addresses registered must remain within the practice area. Check the Area Now.

You do not need to apply to use this service specifically, as access will be given automatically to those patients accepted for other on-line services. Only patients aged 16 years or over can use this service.

We have devised a questionnaire for you to update your doctor on your health. The questionnaire will change from time to time to reflect recommendations from the NHS and other bodies, include questions on specific health conditions which the doctors would like to assess, and will also take into account local health issues and initiatives.

Please take the time to submit the questionnaire as often as you think is appropriate, and watch for new versions coming out. Your answers will be assessed relative to your own needs, your health record will be updated with your answers where they are relevant to your care, and if needed you will be contacted by a clinician for a review. We will need to identify you so it is not possible to submit this anonymously. If you wish to protect your identity when submitting on-line (see Disclaimer, above) contact the surgery first for your personal passcode.

In line with the government agenda to involve patients more in their own care opinion questionnaires are becoming more common, with 2 annual national surveys issued in recent years. With rapid developments elsewhere in the NHS impacting on general practice patient views are becoming more important.

We have devised a Service Questionnaire which may be submitted by patients throughout the year. This will be updated from time to time to reflect current needs and may therefore include a number of different topics. The questionnaires may be submitted anonymously.

From time to time an on-line suggestion form will be available, and may be used to make suggestions on any topic. Each suggestion will be reviewed however it will not be possible to offer any feedback on the outcomes. It will not be possible to use this facility anonymously (see Disclaimer above).

Check the practice area first, and Register on-line. Registration is subject to a validation check and acceptance by the Partners. Shortly after registration you will be invited for an initial health check which provides the doctor with a valuable “base-line” assessment of your health and your needs.

In addition to submitting an application online, copies of the registration form are also available in both Word Document and PDF format so that they can be printed and handed in at reception (see below).
